Millions Face Loss of Health Insurance as ACA Benefits Expire
With the Affordable Care Act (ACA) subsidies set to expire at the end of 2025, millions of Americans are facing the risk of losing their health insurance. These concerns remain significant, especially since Republican control of Congress after the election could pave the way for the expiration of the revised subsidies that significantly lower the cost of health insurance purchased in the ACA marketplaces.
After the November 5 election, where Republicans regained control of the Senate, the stage is set for possible changes to the ACA. Experts and the Congressional Budget Office have warned of the unsustainable impact this could have on the cost of health insurance for many Americans. The revised ACA grants, enacted under the Biden administration, have made health insurance more affordable by lowering premiums, but without renewal from lawmakers, these benefits will expire.
During his last term, President-elect Donald Trump had pushed for the ACA to be repealed, and it remains to be seen what his stance on the program will be when he returns to office. Meanwhile, Republican House Speaker Mike Johnson has urged the need for drastic reforms. He said, “ACA is entrenched, we need big changes to make this work, and we have many ideas to do this.”
But who exactly benefits from these improved subsidies? Americans with incomes more than four times the federal poverty level, which is currently $103,280 for a family of four, are eligible for this financial assistance. Organizations such as the Kaiser Family Foundation (KFF) have reported significant savings for eligible individuals – estimated enrollee fees have fallen by nearly 44% year-over-year, translating to $705 less this year each one. If these subsidies were to end, Americans living in states with high health insurance premiums, especially in rural areas, could find the cost of coverage quickly exceeds their income.
Cynthia Cox, vice president and ACA policy researcher at KFF, noted, “They could go from paying more than 8.5% of their income to easily paying 20% โโor more. Many of those people drop out completely.โ The concern is especially great for middle-income families whose financial stability depends on these enhanced subsidies.
Currently, statistics show about 93% of people who get health insurance through the ACA marketplaces benefit from these improved subsidies. Expiration of these subsidies could cause these people to have higher costs, meaning many may choose to forgo insurance altogether. The enrollment of KFF projects could drop significantly from 22.8 million to 18.9 million if these financial supports are not extended, dropping further to around 15.4 million by 2030.
Currently, these reforms will remain in effect until the end of 2025. Louise Norris, a health policy analyst, confirmed, “If people sign up now during open enrollment, their process will begin to work in January, and will cover them for the whole year. Their rates will not change until 2025.” a 2025. Cox highlighted how enrollment has increased in the past few years, doubling since the boosted subsidies from the American Rescue Plan Act. A large number of low-income enrollees are could be put at risk of losing their health coverage in full.
If these enhanced subsidies are made permanent, they are estimated to cost $335 billion over the next decade. Debates about funding could spark heated debate again as lawmakers grapple with the future of health insurance for millions. The looming shadow of the ACA’s future looms, especially as the country prepares for transformative policy decisions about health care pricing.
With very high numbers for millions, the next few years will be important for the American health system as it deals with the challenges of political changes and aid policies. It raises the question of what can be done to maintain access and affordability for Americans who already rely on these improved subsidies for their health needs.
